چکیده

A woman born in 1912 (Case 8 in the necropsy series) had an unremarkable family history. In 1940, she first noted blanching and cyanosis of the fingers on exposure to cold. Since 1960, small, hard, subcutaneous nodules developed at different locations on her body. Early in 1968, progressive effort dypsnoea and anginal chest pains started insidiously, and treatment with digitalis, furosemide, and potassium was instituted. In September, 1968, she was admitted to the Medical Clinic for heart catheterization. Anticoagulants, reserpine, and theophylline were added to her regimen, but her condition gradually deteriorated, with fatigue, dyspnoea at rest, anginal pains, ankle oedema, cyanosis, and syncopal attacks. She died on January 26, 1969.

Plasma endothelin-1 levels in patients with systemic sclerosis: influence of pulmonary or systemic arterial hypertension.

OBJECTIVES To investigate the behaviour of circulating endothelin-1 (ET-1) in patients affected by systemic sclerosis and to elucidate the relationship between systemic and pulmonary plasma peptide and arterial pressure levels. METHODS Plasma ET-1 concentrations were determined in 48 patients affected by systemic sclerosis (41 women, seven men; mean age 47.2 (SD 5.5) years) with or without sy...

The Pulmonary Manifestations of Generalised Scleroderma

Two aspects of generalised scleroderma have received much attention in recent years. Firstly, the widespread and sclerotic nature of the disease process has been embodied in the term “progressive systemic sclerosis,” proposed by Goetz’ in 1945, and since adopted by other authors. Secondly, connective tissue alterations are now regarded as an integral part of this entity which answers all the cr...

Connective tissue disease-associated pulmonary arterial hypertension

Although rare in its idiopathic form, pulmonary arterial hypertension (PAH) is not uncommon in association with various associated medical conditions, most notably connective tissue disease (CTD). In particular, it develops in approximately 10% of patients with systemic sclerosis and so these patients are increasingly screened to enable early detection. The response of patients with systemic sc...

Serum VEGF levels are related to the presence of pulmonary arterial hypertension in systemic sclerosis

BACKGROUND The association between systemic sclerosis and pulmonary arterial hypertension (PAH) is well recognized. Vascular endothelial growth factor (VEGF) has been reported to play an important role in pulmonary hypertension. The aim of the present study was to examine the relationship between systolic pulmonary artery pressure, clinical and functional manifestations of the disease and serum...

The CREST syndrome--successful reduction of pulmonary hypertension by captopril.

A patient with severe pulmonary hypertension due to pulmonary arteriolar obliteration complicating a long standing CREST variant of systemic sclerosis showed improvement in cardiac output and exercise tolerance during treatment with captopril, which was discontinued because of persistent systemic hypotension. It is suggested that captopril would have a useful and possible prophylactic effect ea...
